Area Agency on Aging - Case Aide
The case aide provides assistance to the Care Management and performs assessment duties, providing a wide range of services to older adults and younger individuals who are eligible for a variety of programs, offered by the PA Department of Aging. An employee in this classification would perform FED's (Functional Needs Assessment tool) in the community. They would also assist Care Management staff with file maintenance, correspondence with consumers, families and physicians. Assist with Community events, Quality Assurance activities, scheduling of appointments, Intake & Referral activities, Taking Reports of need, as well as covering phones as needed. May also manage a case load of home delivered meal consumers if needed. Supervision is received from aging office supervisor.
Bachelor's Degree with a gerontology major or related social science degree such as psychology, social work or nursing: and 1 year experience working with the Aging population preferred.
